Membership Exclusions and Limitations:
- This program is a discount plan, not a dental insurance plan.
- Membership cannot be used in conjunction with another dental plan
- For services for injuries covered under worker’s compensation
- For cost of dental care which is covered under automobile or medical insurance
- For services rendered at a specialist made through a referral
